Level 2 Support Coordination

Level 2 Support Coordination is the most commonly funded level of support, and it’s where most Support Coordinators operate. This service is designed to empower you with the skills to understand, implement, and maximise the use of your NDIS plan.

A Support Coordinator (also known as a COS – Coordinator of Support) will help you build a strong, effective mix of NDIS-funded and mainstream supports to work towards your personal goals—whether that’s improving relationships, living independently, or accessing community and employment opportunities.

Level 3 Specialist Support Coordination (Case management)

Not all Support Coordinators provide Level 3 Support Coordination. This specialised service is delivered by Specialist Support Coordinators —professionals with an allied health background in areas like psychology, occupational therapy, or social work, or experience in other areas such as mental health, palliative care, justice, homelessness, community services, child protection services and case management.

This level of coordination is typically funded for individuals with complex needs who require specialised support or are experiencing a crisis situation. It is often provided for time-sensitive issues or cases where there are significant barriers to accessing services.

Sandy Lewis

Level 2 and Level 3 Specialist Support coordinator

Sandy has been a dedicated Support Coordinator since July 2021, bringing extensive experience in social services. In September 2022, she expanded her expertise to provide Level 3 Specialist Support Coordination, assisting individuals with complex needs.

Holding a postgraduate degree in Social Science, Sandy has worked across various government and community sectors, including the Department of Housing, NSW Police, and the Department of Communities and Justice. Her diverse background equips her with the knowledge and skills to navigate complex systems and provide tailored support.

As a Specialist Support Coordinator, Sandy is committed to understanding each participant’s unique circumstances, strengths, and goals. She takes a person-centered approach, developing practical plans to empower individuals in achieving their aspirations. Whether working with children, adults, or seniors, Sandy provides expert guidance across a range of challenges—helping participants access the right supports and build a strong foundation for their future.

April Almond

Level 2 and Level 3 Specialist Support coordinator

April holds a Master’s degree in Social Change and Development (2016) and a Bachelor of Development Studies (2012) from the University of Newcastle. She commenced in her support coordination role in 2023, bringing valuable experience from her background in Case Management within the Workers Compensation sector.

Passionate about helping individuals develop their skills and achieve their goals, April takes a person-centered approach in her work. She thrives on understanding the unique needs of each participant and crafting individualised plans to provide support through life’s many transitions.

April enjoys working with both formal and informal supports, fostering connections within the community to ensure participants have access to meaningful relationships and resources. She strongly believes in the power of social connection and its impact on overall well-being.

Beyond her professional life, April loves reading about true crime, bushwalking, and hiking mountains—always embracing the adventure, just as she does in her work with participants.

A Support Coordinator helps you understand and use your NDIS plan. We connect you with the right providers, explain your funding, and make sure your supports are working well for you.

If your NDIS plan includes “Support Coordination” funding, you can choose us as your provider. Not sure if you have it? We can review your plan with you and explain your options.

The role of a support coordinator is to:

  • Identify your support needs and connect you with the right services, such as support workers or therapists

  • Help you understand and negotiate service agreements with providers

  • Guide you through your NDIS plan, explaining your funding and how to use it effectively

  • Monitor your budget to ensure you don’t overspend or underspend

  • Link you with community services like support groups, recreational activities, and events

  • Communicate with service providers on your behalf, if requested

  • Assist with resolving provider issues and switching providers when needed

    Explore housing, employment, and financial management options, including self-management, plan-management, or agency-management

  • Prepare you for your NDIS reassessment and attend planning meetings with you if required.

    At Leading Edge Support Coordination, our goal is to make sure you feel supported, informed, and confident in navigating the NDIS, ensuring every service works in your best interest.

Plan Management helps with the financial side (paying invoices, budgets, reporting). Support Coordination helps you actually use your plan to access the right supports and services. Many participants have both.

Like Level 2 Support Coordination, Specialist Support Coordinators assist in implementing and managing NDIS plans. However, they also provide additional expertise in specialised areas, including:

  • Complex health needs, requiring coordination with hospitals and healthcare providers

  • Mental health needs, involving communication with community mental health services and hospitals

  • Challenging family situations, such as children in care or unstable family environments

  • Justice system involvement, supporting individuals navigating legal challenges

  • Homelessness and complex housing needs, assisting in securing safe and stable living arrangements

At Leading Edge Support Coordination, we are committed to helping individuals overcome barriers, connect to essential services, and build a strong, supportive network tailored to their unique circumstances.

You can pay for Support Coordination services through NDIS funding if you have an allocation for it in your NDIS plan. Your NDIS plan will tell you how many hours of support you can receive from your Support Coordinator. Here’s how it works:

Ways to Pay for Support Coordination:

  1. NDIS-Funded Support Coordination – If your plan includes funding for Level 2 or Level 3 Support Coordination, the cost is covered through your NDIS plan, meaning you won’t need to pay out of pocket.

  2. Plan-Managed – If your plan is plan-managed, we will send invoices to your plan manager for payment.

  3. Self-Managed – If your plan is self-managed, you’ll pay invoices yourself and claim the cost from the NDIS portal.

  4. NDIA-Managed (Agency-Managed) – If your plan is NDIA-managed, we will claim payment from the NDIS portal.

  5. Privately Funded – If you don’t have NDIS funding for Support Coordination, you may be able to pay privately or ask NDIS for an early plan review to have Support Coordination funding added to your NDIS plan.

  • Travel Costs – Support Coordinators can charge for travel both ways (to and from your location) and may invoice for kilometres traveled. It’s always a good idea to discuss with your coordinator whether you’d prefer support to be face-to-face, over the phone, or via email.

  • NDIS Rates – The current NDIS pricing is:

    • Level 2 Support Coordination (COS) – $100.14 per hour

    • Level 3 Specialist Support Coordination (SSC) – $190.54 per hour

  • Billing Increments – Our Support Coordinators charge in 6-minute increments, following industry best practice.

  • Flexibility & Reassurance – Whatever your NDIS goals may be, your Support Coordinator is here to help you navigate the system with confidence and clarity.

  • Changing Support Coordinators – If you feel your current Support Coordinator isn’t the right fit, we may be able to allocate you to someone who is better suited. It’s important to understand the notice period required for a transition if you wish to end our service. We ask for 2 weeks’ notice and will assist with a comprehensive handover to ensure a smooth transition to your new coordinator.
